Kaiser Permanente Medical Center- Fontana and Mountain View Centers has signed agreement that Geriatric Fellow Doctors will participate in care of Kaiser Patients under supervision of a Kaiser Permanente attending physician.
Benefits to Kaiser Patients:
Kaiser Doctor visit Mountain View Centers monthly. Therefore care plan is reviewed and monitored regularly. Family can still continue to take loved ones to Kaiser’s primary physician at Kaiser clinic. There is no change in medical care. Only benefit is that if family chooses not to transport loved ones to Kaiser clinic then regular medical care is still attended at the facility. Computerized report will give update to patient’s Primary Doctor.
Kaiser Doctor is available 24/7 on the phone for patient’s medical concern. The result is that we do not need to send our residents to emergency often. It helps patients and family. It has saved life also.
Mountain View is the only assisted living facility in Fontana region where Kaiser Doctor will provide care at the facility. In the past Kaiser Doctors were available at the Skilled Nursing Homes only. Now patients have assisted living option.
There is no cost to patients.
